$25 Sports Physicals

• Cost: $25. Cash, check, or card only. No insurance billing.
• Athlete must wear athletic attire (no flip flops, boots, or jeans).
• KSHSAA Physical Form must be filled out by PARENT and ATHLETE prior to arriving.
• Parent must be present with athletes 8th grade or younger.
• Vaccination history and well child visit check is included.
• ImPACT testing will be available.
