Each November, we take a moment to recognize the Amberwell Health at Home team during National Home Care and Hospice Month. This team provides care and support to patients and families wherever they call home—helping with recovery, managing health conditions, and offering comfort through hospice care.
The Health at Home team recently welcomed back Jennifer Klepees, who has returned to Amberwell as the Director of Health at Home. Jennifer brings valuable experience to the team and is committed to improving care for patients and families.
The group also has a new custom team shirt. It’s a simple design that represents the work they do and the pride they take in caring for others. Watch for posts throughout the month featuring our Health at Home teams wearing their shirts!
Health at Home at Amberwell includes nurses, physical therapists, physical therapy aides, occupational therapists, and occupational therapy aids, who work together to help patients regain strength and independence. Home health aides provide personal care and monitor patients’ well-being, while our medical social worker connects families with resources, financial guidance, and long-term care planning. Office staff make sure everything behind the scenes runs smoothly so caregivers can stay focused on their patients. Each member’s contribution to the team ensures our homebound patients receive the support they need right where they’re most comfortable.
As we head into the holiday season, Amberwell’s hospice care is offering its annual Remember Me Tree Ornament, a meaningful way to honor loved ones who have passed. In addition, the Remember Me Tree Ceremony will be held on November 30 at 6pm at Amberwell Atchison, giving families and friends a time to remember and reflect together.
