Amberwell Health at Home provides a wide range of home health, hospice, and community services wherever our patients call home. Amberwell Health at Home’s patient census has continued to grow each year, allowing them to provide care to an increasing number of our community members. Due to COVID-19, the need for healthcare at home has increased significantly. Amberwell Health at Home has been able to continue to provide excellent and safe care for patients right in their own homes.

Impact on Patient Care
The Amberwell Health at Home team has the unique opportunity to go into patients’ homes and provide them with individual, focused care. The relaxed, familiar setting of the home allows our team members to make personal connections with our patients. The Health at Home team works with their patients to meet each patient’s specific goals. They know that no two patients are the same and enjoy meeting the needs of each individual patient.
Services Provided
Amberwell Health at Home provides a wide range of services under three main aspects: home health, hospice, and community service.
Home health provides skilled services in the patient’s home that would require a nurse, physical therapist, occupational therapist, speech therapist, home health aide, or a social worker. They often see patients after an acute illness or hospital stay but also care for patients with chronic conditions. Their main goal is to assist the patient in reaching their optimum level of independence in all aspects of their health.
Hospice services primarily consist of a nurse, bath aide, and social worker but often also include chaplain services, volunteers, and occasionally therapy. Hospice provides end-of-life care for the patient wherever they are—whether it is a family home, a nursing home, or a hospital setting. The hospice team ensures that the patient is kept comfortable and the family is provided the physical and emotional support they need.
Community services include aid services, such as cooking, cleaning, bathing, laundry, errand running, and meal prepping. These are valuable services that often keep the patient at home for as long as safely possible.

Brandi Oom, Director of Amberwell Health at Home
Brandi has served with Amberwell Health at Home for eight years. She loves serving in the community in which she grew up. She especially loves serving with Health at Home because it provides the unique opportunity to care for patients in their home environments. “With the one-on-one interations in a familiar setting,” said Brandi, “we really get to know the patients and their families on a more personal level and that makes a difference in all our lives.”
About Brandi
Brandi lives in Atchison County with her husband and their four sons, whose ages range from two to thirteen years old. Her older sons are very active in sports and keep her and her husband running to and from events year round.
In her spare time, Brandi enjoys spending time outside, planting flowers, playing with her kids, swinging on the porch swing with her husband, or having movie night with her family. She also teaches Sunday school at her church.
