Kansas requires school sports physicals before students can even start practicing. Get your physical done now instead of waiting until the fall!
Important Information
- Athletes must wear athletic attire. No boots, flip flops, or jeans.
- KSHSAA Physical Form must be filled out by PARENT and ATHLETE prior to arriving.
- $25 per student, paid with credit card, cash or check only (no insurance billing).

You can schedule a same-day or next-day physical at one of our convenient Amberwell Primary Care locations Those who visit the clinic may bill insurance for the physical.
