Each November, we celebrate the Amberwell Health at Home team during National Home Care and Hospice Month. They do important work, helping patients get the care they need in the comfort of their own homes.
The team is led by our administrative director and manager of clinic nursing, who keep things running smoothly and help everyone stay focused on providing excellent, reliable care. Supporting them are the office assistant/building coordinator and office coordinator, who make sure things like phone calls, mail, and supplies are taken care of so the rest of the team can focus on the patients.
One thing that stands out about this group is how involved they are in the community. They’re not just sitting in the office—they’re out there, connecting with patients and families, and taking part in local events. It’s a big part of what makes their work so valuable.
The team includes registered nurses, physical therapists, occupational therapists, and speech therapists. The nurses handle tasks like wound care and IV therapy, while the physical therapists help patients with mobility and recovery. Occupational therapists work on getting patients back to their regular activities, and speech therapists focus on communication and swallowing issues.
Home health aides and medical social workers also play a big role. The aides help with personal care and keep an eye on how patients are doing, and the social workers connect patients with community resources, help with financial matters, and assist with long-term planning.
Whether it’s home health or hospice care, this team is there to support patients and families wherever they are—at home, in a nursing facility, or in assisted living. They provide care during all stages of recovery and illness, making sure people are comfortable and supported through it all.
At Amberwell Health at Home, the focus is always on providing high-quality care where patients feel most comfortable. This month, we’re recognizing each team member for the important role they play and their commitment to our community. Their work makes a real difference, and we’re grateful for all they do!
