Amberwell’s Health Information Management (HIM) team collects, analyzes, and protects patient medical records to ensure their quality, accuracy, completeness, and security.
The HIM department at Amberwell can help you by;
- Faxing continuation of care records
- Sharing images to other facilities
- Copying images to CD
- Assisting you with access to myAmberwellHealth.org
How to Request Patient Medical Records:
Patients who would like access to their medical records for personal use or continuation of care can:
1. Easily download medical records from the patient portal.
The patient portal can be found here: myAmberwellHealth.org
- Once logged into the patient portal, the entire medical record can be downloaded, or an individual visit can be downloaded.
2. Or patients can email the HIM department at healthinformation@amberwellhealth.org.
- Please include your name, date of birth and the type of records you are requesting.
3. Or patients can complete the digital release of information form here: Online Medical Records Release Authorization
- This form will be submitted to the HIM department. Once the records are ready, they will be delivered in the manner specified on the request.
Deceased Patients:
If you are an executor or an administrator of an estate, you may complete the release of information form. The HIM department will request a copy of the death certificate prior to fulfilling any request for records.
Non Amberwell Facilites:
If you are requesting records on a mutual patient for continuation of care purposes, please fax a request on letterhead to 913-674-2011. Please include the patient’s name, date of birth and records requested.
