Amberwell’s Health Information Management (HIM) team collects, analyzes, and protects patient medical records to ensure their quality, accuracy, completeness, and security.
The HIM department at Amberwell can help you by;
- Providing access to your medical records
- Faxing continuation of care records
- Sharing images to other facilities
- Copying images to CD
- Assisting you with access to myAmberwellHealth.org
Medical Records Fees
Amberwell charges a small fee for preparing physical copies of medical records or images on CD.
Electronic access through the patient portal is always available at no cost.
Fee Schedule
Printed records (0–10 pages): No cost
Printed records (11–14 pages): $1
Printed records (15+ pages): $1 plus $0.25 per page
Images on CD: $5
Mailing: $3 flat rate, or exact cost if higher
