How to Request Medical Records from Amberwell
Requesting medical records for your personal use, continuation of care, or as an authorized representative, please choose one of the four convenient options below to request medical records.
Option 1 – Patient Portal
- Visit the Amberwell Patient Portal.
- Log in to download your full medical record or individual visit records.
Option 2 – By Email
- Email our Health Information Management (HIM) department at healthinformation@amberwellhealth.org.
- Include:
- Your name
- Date of birth
- Type of records you are requesting
Option 3 – Request Online
- Complete our Online Medical Records Release Authorization.
- Your request will be processed, and records will be delivered as specified in your request.
Option 4 – Printed Request
If you would like to request your medical records in person or by mail, a medical records release form must be completed and provided to the HIM department. Please use the link below to print the form and follow the instructions:

Requesting Records for Deceased Patients
If you are the executor or administrator of an estate:
- Complete the printed Medical Records Release Form (see instructions above)
- Provide a copy of the death certificate to the HIM department
For Non-Amberwell Facilities
For healthcare providers requesting records for a mutual patient:
- Fax your request on official letterhead to 913-674-2011 for Atchison locations or 785-742-6539 for Hiawatha locations.
- Include the patient’s name, date of birth, and the specific records needed.
